Delivering Exceptional, Patient-Centred Care at Home

Join our dedicated SFH@Home team and play a vital role in transforming patient care through our innovative IV Therapy at Home service, incorporating OPAT, MDCU, and Virtual Ward pathways. This is an exciting opportunity to make a real difference by supporting patients in the comfort of their own homes, ensuring safe, evidence-based, and holistic care.

As part of this dynamic role, you will receive comprehensive training and work within a robust competency framework to develop advanced clinical skills. You’ll gain the knowledge and confidence to adapt to evolving service needs and new care pathways, helping shape the future of Virtual Ward care delivery.

Qualifications

Essential criteria
  • Current level one NMC registration
  • Diploma or degree in health/nursing related studies
  • Completion of the IV Package
Desirable criteria
  • Possession of a mentoring course
  • Continuing professional. development/reflective profile

Further Training

Desirable criteria
  • Venepuncture and Cannulation
  • ANTT
  • CVAD
  • AIMS
  • Oxygen management
  • Lone Working

Experience

Essential criteria
  • Experience of working within a clinical environment, providing patient care
  • Ability to convey sensitive information in an empathetic manner to patients, relatives/carers and staff
  • An understanding of how the IV therapy at home and virtual ward works, including knowledge of the virtual ward delivery methods.
  • Criminal Records Bureau clearance in accordance with Trust policy
  • Awareness of personal strengths and weaknesses
Desirable criteria
  • Experience within working in an acute Trust
  • Experience of working within a community setting
  • Demonstrate an involvement in link nurse, or project work
  • Audit or research within the clinical area
  • Supervision of non-registered staff, pre-registration and post basic students.

Contractual Requirements

  • Physically able to perform the full range of nursing duties.
  • Dexterity and accuracy in undertaking clinical skills, use of equipment and documentation.
  • Competent and capable of using medical equipment
  • Continued professional development.
  • Regular attendance
  • Ability to work flexibly as the service require
  • Hold a full driving licence and have access to a car for business use
