If our purpose and themes resonate with you as a professional, and you would like the opportunity to join our District Nursing then we want to hear from you. As a team leader, you will lead a team of registered nurses and health care support workers to deliver a clinically effective and high quality district nursing service. You will be responsible for the proactive case management of a wide range of patients mainly of whom will have chronic conditions with complex needs. An understanding of community services and the ability to liaise with a wide range of different agencies, whilst adhering to tight time scales and working under pressure are key attributes for this post.
We are looking for a kind, caring, compassionate Registered Nurse with a post registration district nursing qualification and a passion for delivering high standards of community care.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
You must have appropriate UK professional registration.
