Job overview
· We are seeking a dedicated and dynamic Hepatology Support Nurse to join our multidisciplinary team, contributing to the Liver Health Checks and Hepatocellular Carcinoma (HCC) Surveillance programme. This role is essential in ensuring a smooth and coordinated patient journey for individuals eligible for cancer surveillance after a screening fibro scan in Liver Health Checks clinics.
The successful candidate will manage administrative processes for patients undergoing liver health checks, ensuring efficient follow-up care. They will support the Hepatology Clinical Nurse Specialist (CNS) team by delivering patient-centred care for individuals with liver disease, including assessments, care planning, implementation, and evaluation of routine care.
The role includes independently delivering a nurse-led fibro scan clinic and assisting with day case paracentesis and liver biopsy services, ensuring patient comfort and monitoring during recovery. The Hepatology Support Nurse will collaborate with the multidisciplinary team to provide support, advice, and education to patients and staff, ensuring high standards of care.
Additionally, the candidate will provide comprehensive support for patients and families, staying up to date with hepatology nursing to ensure evidence-based care. Main duties of the job
Patient Care and Support
Work within and support a multi-professional team to deliver the Liver Health Checks and HCC Surveillance programmes.
Monitor patients on pathways.
Provide a single point of access for urgent or specialist patient needs.
Manage data and patient tracking.
Develop and maintain a Patient Tracking List for HCC surveillance patients.
Collaborate with CNS team.
Staff Responsibility
Liaise with MDT, outpatient departments, community teams, GPs, and consultants to coordinate patient care.
Administration
Maintain accurate, confidential patient records in line with Trust and NMC standards.
Organise workload effectively, prioritising tasks as needed.
Monitor and review guidelines and protocols for hepatology patients.
Contribute to developing policies for managing patients with healthcare professionals.
Document care according to Trust guidelines.
Strategic and Service Responsibilities
Collaborate with CNS, Consultant Lead, and Matron to improve service delivery.
Assist in establishing a patient database for audits and service development.
Attend education forums and stay updated on national guidelines.
Organisational Skills
Maintain professional development, including Trust mandatory and clinical training.
Develop methods for data collection and present at governance meetings.
Demonstrate IT competency and maintain accurate record-keeping.
