Job overview

We are looking for dynamic and compassionate clinical leaders to join us as Band 7 Clinical Team Leads within our pioneering Integrated Front Door Team. This innovative service is at the forefront of transforming urgent care delivery, improving patient transitions from hospital to community settings, and reducing system pressures.

The Front Door Service is designed to:

  • Prevent unnecessary hospital admissions at the point of entry and within acute services.
  • Reduce length of stay.
  • Promote proactive, preventive care through early identification and intervention.

This is achieved through close collaboration with health, social care, mental health, and voluntary sector partners.

You will have full access to a robust MDT including consultants, GPs, pharmacists, nurses, therapists, and healthcare support workers. We are seeking someone who shares our commitment to excellence in care and can lead the delivery of Front Door team.

Main duties of the job

As Clinical Team Lead, your responsibilities will include:

  • Providing clinical and managerial guidance within the Front Door Service.
  • Collaborating with UCR, Virtual Ward, Care Transfer Hub, Community Specialist Services, Local Authority, and Acute Trust teams.
  • Ensuring efficient patient management and care transitions.
  • Managing a multidisciplinary team of professionals.

The position is responsible for supporting consistent, person-centred care through the use of clinical judgment and critical thinking to assess and plan.

Training and Qualifications

Essential criteria
  • Current registration with NMC or HCPC without restrictions on practice.
  • Evidence of health assessment/ physical examination qualification.
  • Evidence of post-registration, working towards Master's Level (L7) qualification appropriate to the post or equivalent experience.
  • Demonstrable clinical supervision training or management experience or qualification.
  • Demonstrates continuing professional development within area of urgent and emergency care.

Experience

Essential criteria
  • Significant relevant post registration experience.
  • Demonstrable recent experience in acute, emergency, urgent community response, or admission avoidance services.
  • Experience of supervision or management of junior staff.
  • Excellent interpersonal and Leadership skills.

Knowledge and Skills

Essential criteria
  • Evidence of advanced clinical practice skills including advanced physical assessment, cannulation, phlebotomy, and interpretation of investigation results.
  • Management of patients with long-term conditions, multi- morbidity, complex needs, including mental health.
  • Advanced communication skills, negotiation, decision making and organisational skills, as well as succinct accurate handover.

Personal Qualities

Essential criteria
  • Self-directed autonomous practitioner.
  • Excellent communication skills.
  • Digital proficiency (using technology in healthcare).
  • Flexibility – ability to work across boundaries.
