Job overview

The postholder will work as a New to Primary Care Nurse within the North West Primary Care Network (PCN) ensuring an efficient and effective service for patients. The post holder will be part of a multi-disciplinary team within the PCN, delivering high-quality care and supporting the PCN in meeting objectives set out in the 2025/26 Network Contract Directed Enhanced Service (DES). This role is suitable for an experienced Registered Nurse, entering general practice from secondary care or community. 

The post holder will be responsible for the co-delivery and coordination of a PCN-wide wound care service, providing high-quality, evidence-based care is delivered consistently across the network and that unwarranted variation in practice is minimised. The role will also involve undertaking a range of nursing duties, including the assessment and management of patients with long-term conditions, supporting vaccination and immunisation programmes, and contributing to health promotion and preventative care initiatives. Working autonomously within the primary care setting, the post holder will support key priorities such as proactive and personalised care, population health management, and improved outcomes for patients with long-term conditions. This role offers an excellent opportunity for an experienced nurse to further develop their skills and progress a long-term career in primary care.

Main duties of the job

Co-deliver and coordinate the PCN wound care service, working across our GP practices and with community partners.

Provide expert wound assessment, treatment planning, and reviews using evidence-based practice.

Support consistent documentation and use of standardised wound care pathways and formularies across the PCN.

Act as a point of contact for wound care advice for other clinicians within the PCN, escalating complex cases appropriately.

Contribute to service audits, quality improvement, and data reporting related to wound management outcomes.

Provide fundamental nurse-led care to communities across the PCN.

Undertake early detection, prevention and management of cardiovascular disease and other long-term conditions

Provide vaccination and immunisation support across GP practices and the PCN

Participate in quality improvement initiatives to enhance patient outcomes and reduce health inequalities

Work within an MDT, supporting integrated care and personalised approaches to patient management

Promote preventative care and support patients in self-management through personalised care planning

Participate in multi-agency and multi-disciplinary team meetings related to wound care and general nursing

Actively participate in clinical and practice meetings

Qualifications/ Knowledge/ Experience

Essential criteria
  • NMC/UKCC registration
  • Extensive knowledge of wound care or willingness to undertake specialist training
  • Experience in reviewing, assessing and interpreting risk factors relating to Chronic Disease management
  • Competent in all Practice Nurse duties
  • Can provide evidence of CPD
  • At least 3 years’ experience working as a qualified nurse
  • Hold a recognised Immunisation and Vaccination qualification in line with UKHSA guidance, or be willing to undertake
Desirable criteria
  • Doncaster Wound Care Alliance training or equivalent
  • Tissue viability specialist nursing
  • Prescribing qualification
  • Knowledge of primary care structure and PCN service delivery
