Job overview

·         The post holder will work as a member of a small, dedicated clinic providing assessment and treatment to patients experiencing mental health difficulties of a serious and enduring nature.

 to be responsible for a caseload of patients who require assessment and monitoring as part of their ongoing treatment within secondary mental health services.

 To provide support and guidance to junior staff members working into the Clozapine and Lithium Clinic where required.

   To work as part of a wider system of care provision to ensure patient journey is as seamless as possible.

 To provide professional advice and support to other agencies and members of the multidisciplinary team.

Main duties of the job

Assess, plan and review care and treatment needs including risk assessment and management.

Provide a duty response for your caseload where possible to support with more urgent care needs.

Work as part of a small but dedicated team alongside wider mental health teams to ensure that patients receive the highest standard of care and treatment, using evidence-based practice.

Provide effective advice, guidance and support to patients, carers/relatives, other professionals in relation to care and treatment.

Work to professional responsibilities at all times, adhering to the NMC Code of Conduct at all times.

Work under the requirements of the Mental Health Act 1983 where required. 

To promote the engagement of carer and patient involvement within the service.

 Establish therapeutic relationships with patients, whilst maintaining professional boundaries in accordance with professionals code of conduct. 

Ensure clear lines of communication with patients, carers/families and other professionals are maintained at all times.  Documentation should also be of the highest standard.   

To make appropriate onward referrals to services that can support patients and or family members/carers where required.

Providing appropriate support and caseload cover for colleagues/team members where required.

Qualifications

Essential criteria
  • Registered Mental Health Nurse (RMN).
  • Evidence of continuing personal learning and development.
  • Mentoring qualification.

Experience

Essential criteria
  • Experience of working in mental health services and managing risk.
  • Experience of providing education to various and multi-professional groups
  • Experience of providing education to various and multi-professional groups

Knowledge

Essential criteria
  • Leadership and management skills.
  • Good communication, both written and verbal.
  • Knowledge of contemporary practice and best practice.
  • Ability to manage, supervise and support staff.
