The candidate should have knowledge of the NHS Framework for Continuing Healthcare.

The successful candidate will provide specialist assessment and review for adults and older people presenting with long-term care needs who may be eligible for NHS Continuing Healthcare, NHS-funded nursing care and jointly funded packages of care in line with the NHS Framework for Continuing Healthcare.

Experience of undertaking assessments, and working with patients, families and carers is essential. The Personalised Healthcare Team are responsible for the assessment and review of people who reside in nursing care homes and the clinical commissioning of packages of care for people with continuing healthcare needs who reside in their own homes.

Ability to transport yourself (drive and hold a clean, valid UK driving licence with appropriate insurance)  over a large geographical area whereby the service requirements are fully met.

Main duties of the job

To provide specialist nursing assessment and review for Learning Disability adults and older people presenting with long term care needs who may be eligible for Continuing Health Care, Funded Nursing Care and jointly funded
packages of care. To provide assurance around quality of commissioned care packages and reporting of safeguarding concerns in accordance with agreed interagency reporting procedures.

Detailed job description and main responsibilities

Co-ordinating assessment for NHS Funded Nursing Care and Continuing Health Care as close to the service user as feasible.

To provide clinical commissioning oversight to applications and assessments concerning joint funded packages of care.

 On-going review of service users eligible for CHC, NHS funded care in accordance with National Framework for NHS Funded Care, and Joint funded care packages of care.

Undertake urgent reviews of service users, where quality and safety concerns have been identified and report in a timely manner.

To prepare/complete appropriate documentation/tools to enable the level of NHS funding to be determined within the parameters of the National Framework for Continuing Health Care and Funded Nursing Care and Joint Funding arrangements locally.

To provide assurance with regard to quality of care of individual packages by undertaking reviews as agreed;

To actively promote communication and partnership working between the statutory, independent and voluntary sectors.

 To communicate relevant information and to provide advice and support on funded nursing/continuing health care to service users and carers.

 To liaise with other health professionals on clinical matters as part of the assessment process.

To maintain professional confidentiality in all matters relating to patient care.

Qualifications

Essential criteria
  • Current UK NMC Registration for either RNLD or RMN.
  • Current, full, UK driving licence.
