Job overview
The post holder will be a registered healthcare professional, be an independent non-medical prescriber and practice autonomously. Incorporating physical assessment, diagnostic testing and interpretation and formulation of treatment plans whilst avoiding hospital admission.
The post holder will provide clinical leadership to the Virtual Ward Service on a daily shift basis when allocated. The role will provide clinical leadership and offer advice to the wider virtual ward team. In addition to providing ongoing supervision and support participating in the induction and training of the multidisciplainary team.
The post holder will initiate, manage and drive change in practice for the benefit of patient care in line with current trust and local programmes and in association with the NHS Long Term Plan, in particular supporting patients care closer to home as part of admission avoidance and virtual ward care/hospital at home
You are required to hold a valid and in-date driving licence and have access to a car

Main duties of the job
The post holder will manage the Acute Virtual Ward ensuring beds available against agreed pathways to deliver a 7 day 8-10pm service on a day to day basis.
A list of main tasks and duties are outlined below but are not exhaustive due to the evolving nature of this unique role and service.

Establishes and maintains effective working relationships, both internal and external
in order to meet the requirements of the service.
Demonstrates clinical expertise and leadership qualities on a daily basis in order to
support the overall running of the service whilst supporting the Virtual Ward Matrons.
Confidently lead on ward rounds, daily huddles and multidisciplinary team meetings
Prescribing and deprescribing of medications for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence-based practice and national and local protocols and within the role’s scope of practice and legal framework
Is able to take the lead in complex case management, providing support and advice to others
