Our CYPS Community Team in Northeast Hampshire (covering Hart & Rushmoor) is a highly supportive, friendly, and innovative service. Guided by the I-THRIVE framework, we focus on providing Getting More Help and Risk Support to children and young people. As an urgent care practitioner, you will play a pivotal role in providing timely assessments to children and young people, ensuring high standards of practice, and fostering collaboration across the multidisciplinary team.

  • Professional registration with NMC or HCPC.

  • Significant experience working with children and young people under 18 with moderate to severe mental health needs.

We are seeking an experienced RMN, OT or Social Worker to provide urgent assessments within the 10-day time frame and short-term goal based interventions either individually, or with a member of the multi-disciplinary team depending on the complexity of referral within a community setting.  The post holder will be able to offer a range of therapeutic interventions appropriate to the care pathway.

 Management of referrals including screening and delivery of care (where appropriate)

·       Supervision of clinical junior staff.

·       To support the NEH Hub in the delivery of discovery conversations, urgent  assessments, short-term treatments and groups where required.

Detailed job description and main responsibilities

This role requires a high level of skill in crisis management, risk management, safeguarding and liaison with other agencies, professional and voluntary.  The role requires the ability to operate with a high degree of professional autonomy but work collaboratively with other members of the team and wider network to formulate and treat children, adolescents, and their families.

 Key Responsibilities 

       Carry own caseload and undertake holistic assessments including risk of the child/adolescent/family needs, either individually or in identified complex cases jointly with another member of the CYPS Community Team.

       Provide short term treatment to CYP. 

 To liaise with Social Care, Education and other appropriate agencies/professionals /carers to ensure best outcomes for CYP.

 Act as a lead professional for CYP following initial assessment. Developing appropriate plan of care.

       Offer support for Students on placement.

       Co-work with other members of the MDT.

       Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies and Professional body requirements.

         Contribute to the team’s overall standard of services provided.

      Promote effective liaison with all statutory and voluntary agencies concerned with CYP and mental health.

       Through the appraisal and supervision systems identify own training needs and undertake continuous professional development.

Qualifications

Essential criteria
  • Degree in Nursing, Social Work or Occupational Therapy
  • Registration with NMC, HCPC or SWE

Experience

Essential criteria
  • Experience of robust risk formulation and management
  • 2-year post qualification experience of working in the community with young people with mental health difficulties and their families.
  • Demonstrable expertise in complex assessment of people with mental health needs
Desirable criteria
  • Awareness of National agendas for the development and growth of CYPS ND
