This unique position offers you the opportunity to work as part of a highly skilled community workforce who are the cornerstone of safe patient care. You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions. We have lots of training and development pathways at every level through from band 3 to band 8. We provide a preceptorship program to support you in your role and ongoing training and development.

We use electronic clinical records through EMIS and a work IPad and IPhone will be provided.

 

 

Main duties of the job

We are seeking a highly motivated and ambitious Senior Registered Nurse to join the wider Community Nursing Team which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates and Healthcare Assistants. You will work under the direction of a District Nurse to assess, implement and evaluate person centred care for a caseload of patients. You will undertake clinical skills including venepuncture, wound care, catheterisation and end of life including syringe drivers.

 

Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Detailed job description and main responsibilities

The post holder will ensure that patients, services users, families and carers have a positive safe and high quality experience when accessing and receiving care, treatment, advice and support from Procare employees and will ensure learning is shared and embedded in situations where things did not go as well as they should have.

To deliver nursing care in accordance with national and local policy and guidance and in accordance with the Nursing and Midwifery Council.


The post holder will work under the direction of the District Nurse to support the management of a team of nurse, associate practiitioners, health care assistants and phlebotomists ensuring the effective, efficient, safe and high quality nursing assessment, treatment and delivery, and evaluation of nursing care to patients and carers within their own home setting.

Qualifications

Essential criteria
  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Educated to diploma or level 6/ degree in relevant area
  • Supervisor / Mentorship qualification

Knowledge and Experience

Essential criteria
  • Broad range of clinical skills relevant to Community Nursing, - including but not limited to Compression Bandaging, Dopplers, Venepuncture, Male & Suprapubic catheter, Syringe drivers and Palliative care & Symptom Management
  • Experience prioritising and allocating referrals and visits in accordance with patient clinical need
