Job overview

We are looking for an enthusiastic and motivated Band 6 Nurse or District Nurse (SPQ) to join our friendly and supportive Community Nursing Team. This is a fantastic opportunity for an experienced, compassionate nurse who enjoys leadership, values teamwork, and is passionate about delivering outstanding care to patients in their own homes.

As a Community Nursing Sister/Charge Nurse, you will have a key role in managing a caseload of housebound patients with complex needs, ensuring care is safe, effective, person-centred and evidence based. Working closely with the Team Leader, you will provide day-to-day clinical leadership, support and supervision to colleagues, act as a positive role model, and contribute to the ongoing development of community nursing services.

You will bring strong post-registration experience, either within community nursing or from a role with clearly transferable skills. Alongside excellent clinical ability, you will demonstrate effective caseload management, good organisational skills and a commitment to continuous service improvement.

Main duties of the job

    • Support the day-to-day running of the Community Nursing Team in partnership with the Team Leader

    • Ensure the team consistently delivers high-quality, safe and effective patient care

    • Provide leadership, guidance and clinical support to nursing staff

    • Contribute to a 7-day service across North East Hampshire, working within a rota covering 8am–8pm

    You will also work collaboratively with other Band 6 nurses across locality teams as part of a wider, supportive leadership network.

Detailed job description and main responsibilities

  • Manage the daily coordination of the community nursing team.
  • Undertake holistic assessments for patients newly referred to the service.
  • Complete and oversee clinical assessments, including PURPOSE-T, MUST/MUAC and wound assessments.
  • Manage chronic wounds (e.g. leg ulcers and pressure ulcers) in line with NICE guidance and best practice.
  • Manage short-term wounds, including post-surgical wounds.
  • Care for patients with drains (e.g. JVAC, lantern and pigtail drains), including flushes where required.
  • Provide care for PICC lines, midlines and portacaths, including chemotherapy disconnections (training provided).
  • Administer intravenous antibiotics in the community via bolus or infusion.
  • Ensure appropriate pressure-relieving equipment is in place and refer to specialist services as needed.
  • Communicate effectively with GPs and other professionals to support coordinated patient care.
  • Participate in integrated care working, including MDT meetings when required.
  • Advocate for patients and ensure their needs and wishes remain central to care delivery
