Job overview
We are seeking an experienced and motivated Community Heart Failure Specialist Nurse to join our expanding service. This is a rewarding opportunity to make a significant difference to patients living with a confirmed diagnosis of Heart Failure—supporting them to manage their condition safely at home, reducing unplanned hospital admissions, and improving long-term quality of life.
The post holder will provide expert clinical knowledge and specialist nursing skills in the management of Heart Failure across the community setting. You will hold a defined caseload, delivering comprehensive assessment, treatment optimisation, education, and follow-up to both newly diagnosed and existing patients.
Working autonomously—yet fully supported by the wider MDT—you will:
- Provide proactive management of patients to avoid unplanned hospital admission.
- Optimise medication and treatment plans in line with national and local Heart Failure guidelines.
- Deliver patient and carer education to support confidence and self-management.
- Liaise closely with GPs, secondary care cardiology teams, community nursing, therapy services and social care to ensure seamless, coordinated care.
- Advocate for high-quality Heart Failure care in line with NSF standards and contribute to ongoing service development and strategic planning.
Main duties of the job
- Use advanced clinical skills and evidence-based knowledge to identify investigation and/or treatment needs of individuals.
- Provide medicines management support and advice in accordance with national and local guidelines.
- Provide appropriate advice and support to patients and their carers including the provision of education and the promotion of self-management skills.
- To liaise with local cardiac rehabilitation teams and End of Life teams as appropriate.
- Act as a specialist resource for patients, their carers and other health professionals to improve the management of patients with heart failure.
- Liaise closely with primary and secondary care services to provide an integrated care plan in which the patient and carers participate.
- Ensure mechanisms in place for escalation of care at times of clinical deterioration
- To triage referrals from GPs, consultants, inpatient heart failure team and other professionals and service in agreement with individual patient.
- Provide empowerment and support, acting as an advocate to enable individuals and their carers to actively engage in agreed treatment programmes.

Detailed job description and main responsibilities
- Manage and facilitate the care of patients with Heart Failure in the community through outpatient clinics and home visits.
- Providing specialist assessment for individuals with heart failure and co-ordinate individual care plans to maximise health outcomes.
- Use advanced clinical skills and evidence-based knowledge to identify investigation and/or treatment needs of individuals.

- Maintain and further develop expertise within Heart Failure field. To make effective use of learning opportunities as part of continuing profession development.
- To evaluate own knowledge, practice and learning needs as part of KSF, appraisal in professional development plans with line manager.
- To demonstrate an ability to empathetically communicate with all patients and have a willingness to respond to communication of any kind from patients and carers.
- To provide up to date written and verbal communication on patients to other professionals in the hospital, community and across the boundaries.

- Registered Nurse (RN) on Part 1 or 12 or the NMC Register
- Post graduate cardiac qualification
- Teaching, assessment / mentorship qualification
- Advanced physical assessment qualification
- Heart failure qualification

- Counselling qualification

- Post graduate experience
- Band 6 cardiology experience or experience of dealing with heart failure patients
