You’ll work shifts between 8am and 9pm, including weekends, and play a vital role in early identification of deteriorating patients, all while developing your skills through continuous professional development.

Main duties of the job

  • To provide high quality care to clients under the indirect supervision of Team manager within the community bridging service.
  • To support clients with activities of daily living in a clients home
  • Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals
  • To participate in continuous professional development and other team activities
  • Carry out a range of duties within agreed competency skill set e.g. meal prep
  • To ensure that all patients on the caseload are safe.

Qualifications

Essential criteria
  • Level 2 Apprenticeship in Care (or relevant subject) or a minimum of 1yr experience in health and social care setting
  • GCSE maths and English (or equivalent)
Desirable criteria
  • Level 3 Apprenticeship in Care (or relevant subject)
  • Care certificate

Experience

Essential criteria
  • Previous experience in care eg as HCA
  • Safe moving and handling experience
Desirable criteria
  • Previous community experience
  • Complex moving and handling experience

Skills & Knowledge

Essential criteria
  • Moving and handling of clients
  • Personal care to clients in a person-centred manner
  • Follow clear instructions and feedback to senior staff
  • Work safely with minimal supervision
Desirable criteria
  • Knowledge of some medical conditions / exercise therapy
  • Good interpersonal and communication skills both verbal and written
