Job overview
Are you a passionate and experienced community nurse ready to take the next step in your career? We are looking for an enthusiastic Band 7 Community Nursing Team Lead to join our supportive and forward-thinking community nursing service.
This is an exciting opportunity to play a key leadership role in delivering high-quality, compassionate care to patients within their own homes and communities. As a Team Lead, you will work closely with the Clinical Lead and District Nurse to ensure our service remains safe, effective, and truly person-centred.
You will lead and support a dedicated team of community nursing professionals, promoting excellence in clinical practice while creating a positive and supportive team environment. Your leadership will help ensure smooth day-to-day service delivery, support staff development, and drive continuous improvement across the service.
This role offers the chance to make a real difference — not only to the patients and families we care for, but also to the staff you support and develop.
If you are a motivated leader who is passionate about community care, collaboration, and improving patient outcomes, we would love to hear from you.
Come and be part of a team that is committed to delivering outstanding care in the community while supporting each other to grow and succeed.
Main duties of the job
In this influential role, you’ll provide vital support across localities to ensure that safe, high-quality care is consistently delivered within the community.
Working in close partnership with the Clinical Lead and District Nurse, you’ll lead and manage our dedicated community nursing team—overseeing patient care and incidents, championing staff development, upholding professional standards, and actively contributing to clinical governance. Your leadership will be highly visible, offering guidance, support, and motivation to colleagues as you help shape the culture and performance of the service.
This dynamic position blends 40% clinical practice with 60% management, giving you the opportunity to stay hands-on in patient care while driving service improvement and team development.
