The Urgent Out of Hospital Service is seeking an experienced Senior Nurse to join our innovative and expanding team. Bringing together Hospital@Home, Urgent Community Response (UCR) and Virtual Ward services, we are at the forefront of delivering integrated care closer to home, supporting the NHS vision of providing high-quality, patient-centred care outside of hospital settings.
Based at Milford Hospital, our team receives referrals from a wide range of partners across the Guildford and Waverley community, including GP practices, ambulance services, social care teams and voluntary sector organisations. Together, we provide safe, compassionate and responsive care in patients' own homes, helping to prevent unnecessary hospital admissions and supporting people to remain independent for longer.
Main duties of the job
As a Senior Nurse, you will play a key role in delivering urgent and planned care within the community. Working autonomously and alongside a skilled multidisciplinary team, you will undertake urgent two-hour response visits, comprehensive assessments, clinical reviews and care planning to support patients with complex and acute health needs.
You will be responsible for delivering high-quality, evidence-based care while working collaboratively with colleagues across health and social care services. Your clinical expertise, leadership and problem-solving skills will help ensure patients receive the right care, at the right time, in the right place.
A full UK driving licence and access to a vehicle are essential for this role. Business mileage will be reimbursed in accordance with the Trust's expenses policy.

Detailed job description and main responsibilities
You will be part of a fast paced, ever evolving community MDT, where your proven ability to make high level clinical decisions will aid patient recovery. This will include having a direct link to the patients GP, as well as other health care professionals to prevent hospital admission.
You will provide clinical leadership within the Urgent Community Response (UCR) and Hospital at Home teams, delivering high-quality, patient-centred care to prevent unnecessary hospital admissions and support early discharge. The role includes managing day-to-day service delivery, coordinating and prioritising caseloads, supervising and supporting staff, ensuring safe staffing and skill mix, and contributing to quality improvement, governance, and service development in line with organisational standards.

Qualifications
Essential criteria
- NMC registered qualification
- Practice supervisor/Practice Assessor
- Evidence of a range of clinical competencies appropriate for the role.
- Degree level qualification in relevant training or equivalent knowledge and experience in health related subject
Desirable criteria
- Relevant urgent care, community, frailty or long term conditions related training
- Health Assessment course
- Non -Medical Prescriber
Knowledge and Experience
Essential criteria
- Experience of delivering care in acute, community, intermediate care or urgent care setting
- Experience of working independently whilst recognising professional boundaries
- Evidence of CPD aligned to the role
- Understanding of integrated care systems and community pathways
- Leadership and supervision skills
Desirable criteria
- Experience in urgent community response, virtual ward, frailty or admission avoidance services
- Knowledge of the national UCR services specifications (2 hour response model)
