Job overview

We are looking for a Community Wound Care Nurse to join our  Community Nursing team.  

This unique position offers you the opportunity to work as part of a highly skilled community workforce to deliver high quality advice and assessment for patients with complex or long term wounds. The role will drive forward quality improvement to enhance patient outcomes and lead initiatives focussing on lower limb care. 

We use electronic clinical records through EMIS and a work IPad and IPhone will be provided.

This role is suitable for experienced community registered nurses. 

 

Main duties of the job

You will provide high quality, specialist care to housebound patients with complex wounds. This service additionally provides advice and support to the Community Nursing Teams as required, as a source of expertise in managing complex wounds.

As an experienced Wound Care Community Nurse, you will be able to provide sound clinical judgement and act as a role model to junior staff and students. You will be taking responsibility for assessing complex patients and planning their care needs.

We have a variety of shifts available Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Detailed job description and main responsibilities

As an experienced community nurse the post holder will be embedded in the Guildford and Waverley Community Nursing team assisting in delivering routine planned care such as medication management while having a focus on advice and assessment to those patients who have complex or long term wounds, including lower limb wounds. They will work alongside community nursing staff to ensure skills are gained and maintained, increasing competence across the teams. They will ensure that patients with complex or long term wounds have a comprehensive holistic review and that care plans reflect evidence based practice.

Qualifications

Essential criteria
  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Post Graduate Tissue Viability Qualification or willingness to undertake within 18months of being in post if post continues
  • Evidence of BSc level / level 6 study
  • Assessor / NMC Supervisor / Mentorship
  • Evidence of recent and relevant continuing professional development or experience in wound care

Knowledge and Experience

Essential criteria
  • Holistic lower limb wound assessment and management qualification - Handheld / automated Doppler
  • Assessment and Compression bandaging competencies
  • Advanced range of Community clinical skills and ability to undertake full assessments and physical assessments of patients including those with multiple pathology and complex health and social needs in the Community
