• ACP (Band 8a): Practise autonomously at advanced level, contributing to clinical leadership, service development and high‑quality frailty care.
• Trainee ACP (Band 7): A structured development role with strong supervision and support, including completion of a Master’s‑level Advanced Clinical Practice programme and clinical portfolio, progressing to Band 8a on achievement of ACP competencies.
