We have an exciting opportunity for a Nurse, Occupational Therapist or Social Worker to join Surrey & Borders NHS Foundation Trust (SABP) as an Urgent Care Pathway Clinician in the Children and Young People Services Community Team based in the East of Surrey.

The Urgent Care Pathway Clinician will be a member of the multi-disciplinary specialist team, providing services to children and young people 5-18 years and young people in transition to adult services with mental health needs, emotional and behavioural problems.

There is 1 full-time post covering the East of Surrey with SABP bases in Redhill and Leatherhead.

Main duties of the job

The post holder will provide assessments and treatment, either individually or with a member of the multidisciplinary team depending on the complexity of the referral, within both clinic and community settings.

The post holder will be able to offer a range of therapeutic interventions appropriate to the care pathway the young person is on.

This role requires a high level of skill in crisis management, risk management, safeguarding and liaison with other agencies, professional and voluntary. The role requires the ability to operate with a high degree of professional autonomy but work collaboratively with other members of the team and wider network to formulate and treat children, adolescents, and their families.

As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations, you will be supporting the team in leading with the implementation and embedding of the THRIVE Framework across the newly created CAMHS Intensive Intervention Team in Surrey.

Detailed job description and main responsibilities

Key Responsibilities
• Carry own caseload and undertake holistic assessments including risk of the child/adolescent/family needs, either individually or in identified complex cases jointly with another member of the CYPS Community Team.
• Provide short term treatment to CYP.
• To liaise with Social Care, Education and other appropriate agencies/professionals /carers to ensure best outcomes for CYP.
• Act as a lead professional for CYP following initial assessment. Developing appropriate plan of care.
• Offer support for Students on placement.
• Co-work with other members of the MDT.
• Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies and Professional body requirements.
• Provide statistical information on all activity undertaken, in line with Trust procedures. Have basic IT skills and Data entry skills to enter clinical information on the data bases.
• Contribute to the teams overall standard of services provided.
• Promote effective liaison with all statutory and voluntary agencies concerned with CYP and mental health.
• Through the appraisal and supervision systems identify own training needs and undertake continuous professional development.

Qualifications

Essential criteria
  • Clinical qualification at Diploma level or above in Mental Health Nursing, Social Work or Occupational Therapy
  • Registration with NMC/HCPC
  • Post qualifications in the areas of child development, attachment, or family functioning
Desirable criteria
  • Post qualification training to degree level

Experience

Essential criteria
  • 2-year post qualification experience of working in the community with young people with mental health difficulties and their families
  • Demonstrable expertise in complex assessment of people with both mental and physical health issues
  • Demonstrable expertise in risk assessment and implementation of risk management plans
  • Demonstrable ability to develop practical innovations and approaches to issues and complex problems
  • Use of outcome measures to evidence good practice
  • Awareness of National agendas for the development and growth of CAMHS
  • Proven ability to provide training and education regarding mental health and complex clinical topics
  • Proven track record of building positive relationships with partner agencies
  • Expertise and experience in supervision and supporting the growth of other staff
