Job overview
Frimley Health NHS Foundation Trust is seeking a highly skilled and clinically motivated Outreach Practitioner to join the Rapid Response Team.
This clinically focused role supports the implementation and consolidation of Martha’s Rule across the Trust and provides expert leadership, education, and clinical support for patients at risk of deterioration, life-threatening events, or cardiac arrest.
The postholder will bring significant experience in managing medical emergencies and in developing and implementing deteriorating patient and resuscitation policies. Excellent communication skills and a strong commitment to patient safety, quality improvement, and the critical care agenda are essential.

Main duties of the job
The Outreach Practitioner will be a key member of the Rapid Response Team, focusing on the early identification and management of deteriorating patients to prevent adverse outcomes, including cardiac arrest.
The role involves providing rapid clinical support and expert decision-making during medical emergencies, contributing to the development and implementation of deteriorating patient and resuscitation policies, and working collaboratively with multi-professional teams to enhance patient safety. The postholder will also deliver education and support to staff on early recognition and escalation of care.

Qualifications
Essential criteria
- Registered Adult Nurse/AHP
- BSc (Hons) in relevant subject or equivalent
- Post registration qualification in Critical Care Nursing or equivalent Acute Care course
- Teaching and Assessing/Mentoring qualification
- ALS provider
- Physical assessment course or working towards at level 7
Desirable criteria
- MSc or working towards
- ALS Instructor or ILS Instructor or working towards
- Non – medical prescribing or working towards
- APLS/EPALS provider
Skills
Essential criteria
- Able to assess, plan, implement and evaluate care with minimal supervision
- Effectively communicate with all levels of the organisation.
- Time management and delegation skills.
- Good IT skills
Desirable criteria
- Counselling skills
Experience
Essential criteria
- Minimum of 3 years Band 6 critical care /Acute Care experience
- Experience in providing formal teaching.
Desirable criteria
- Previous Outreach experience
