Job overview

We are looking for individuals with enthusiasm, initiative and a positive approach to work to join one of our Locality integrated nursing teams in Gateshead as a Health Care Assistant.  You will be responsible under the direction of the nurse in charge to provide high quality care to patients in their own home.   This post includes telephone communication with patients, relatives and professionals and also includes the use of EMIS community WEB the IT system used in community. The ability to communicate positively at all levels will be an essential attribute of the post holder, as well as a flexible self-confident approach to work.

You should possess excellent interpersonal skills and have an NVQ Level 3, or equivalent in relevant subjects or relevant experience. Experience in a health care setting is desirable or evidence of a range of clinical skills. You must also have evidence of experience in Microsoft packages and data bases.  In return for your commitment to team working we will offer you a comprehensive induction and support in personal and professional development within the appraisal process.

 You must be a car owner/driver

Main duties of the job

Support the Locality Team and service leads with the day to day delivery of a range of clinical interventions to patients within the community setting. Support the locality team lead in the development of any new ways if working that will ultimately improve patient care as and when required.  Support the development and delivery of the community transformation Project including the implementation of robust working practices processes, audit and lessons learnt.

This post will be based in one of the community locality teams with an expectation that the post holder may be asked to cover as part of the wider team in another location.

Detailed job description and main responsibilities

  • Assist with delivery of care to patients who are under the locality team , which would include wound care, phlebotomy, covering SPOC, catheter care, general observations, weight supporting qualified staff, ordering of stock and assisting with stock rotation of dressings.
  • Planning own workload with the locality team ensuring timely patient intervention.
  • Ensure accurate and effective records are kept. Assist in maintaining any schedules, appointments, clinics  and visiting schedules as and when required.
  • This post will be based in one of the community locality teams with an expectation that the post holder may be asked to cover as part of the wider team in another location.
  • This post is to cover an 8am until 8.00pm 7 day service where weekend and evening work will be required.
