Community Nurse/ Care Coordinator

Department: Older Persons Community Treatment Team (CTT)      

Location:      Newcastle – based at Centre for the Health of the Elderly, Campus for Ageing & Vitality.

A Community Nurse, Band 5 is required to work in the Older Persons Community Treatment Team. This is a busy team covering all localities across Newcastle and as well as the eastern part of the borough of North Tyneside

We are looking for a suitably qualified person who is dynamic and motivated with a specific interest in the mental health care of the older people. The appropriate knowledge and skills to assess, deliver and coordinate treatment and interventions for older adults with cognitive problems and all other mental health conditions where physical co -morbidity or other age related complexity is required.

The successful applicant must be a confident, experienced nurse with excellent communication and interpersonal skills who is focused and well organized. The demands of the job also requires ability to be autonomous, flexible and responsive with a positive attitude to team work.

All clinical posts are subject to DBS clearance (formerly CRB) prior to commencement.

You will work in a highly supported environment as part of the wider Older persons Pathway and will be provided with support to develop your knowledge and skills. You must have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trusts core Values.
