Endocrine Clinical Nurse Specialist

Job overview

A exciting opportunity has arisen for an individual to contribute towards the provision of excellent patient care within the endocrine service. We are looking to appoint a Clinical Nurse Specialist in Endocrinology to join the Endocrine team at UHCW. You will join a team of experienced specialist nurses in a tertiary Endocrine referral Centre and be given full structured supervision to develop your skills in Endocrinology.

The post holder will play a key role in developing the Endocrine service and you will be expected to perform autonomously both in the investigation unit – performing dynamic function testing and specialist screening/ treatments / procedures and clinics, devloping and managing nurse led clinics, developing audit tools and clinical protocols / guidelines, leading audit / research acitivites, developing and delivering patient / staff education programmes.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

  • Co-ordinate patient care within the Endocrine Services
  • Provide specialist care, advice and support to patients, families, careres and all staff involved in the care and treatment of Endocrine patients with long term health conditions throughout their diagnostic treatment, surveillance and follow-up pathway.
  • Educate nursing, medical and other groups of staff about Endocrine and its management and be available as a resource for these staff groups.
  • Link with members of the Endocrine Team to facilitate continuing care of those with recurrent/long term contitions.
  • Develop a proactive role in health promotion to increase professional and public awareness of Endocrine Disease / long term conditions
  • Undertake clinical audit/service evaluation in relation to specialist practice.
  • Develop specialist nurse led services as required to supprt service development.

Detailed job description and main responsibilities

  • To provide direct, specialised care and advice to patients within a defined caseload, using specialist nursing skills and ensuring that care and support to patients and families is sensitive to their needs for courtesy, dignity and privacy.
  • Provide Nurse led clinical activity to support the Endocrine service across the Trust. This will include running nurse led day case Clinic. Telephone advice and general support.
  • Meet with the patient and, when wished, their family at key stages in their pathway to provide individualised written and verbal information and support that will help them to prepare for treatment and its outcome at key points in their pathway.
  • To act as a patients’ advocate and to give advice regarding treatment options in order that patients are able to give informed consent.
  • Provide on-going support and continuity of care to patients and their families in hospital, clinic and home settings and by telephone contact. Refer to other agencies as necessary – acting as key worker.
  • Meet the patient’s individualised care needs through utilising the holistic assessment process throughout the patient pathway, promoting self- management following treatment.
  • To work in collaboration with all members of the local and Specialist multi-disciplinary teams in order to ensure the provision of a high quality, timely service for patients and their families.
  • Promote and demonstrate evidence-based practice, initiate research/audit projects and encourage and support staff who wishes to be actively involved in service development.
  • Collaborate with multi-disciplinary teams and produce a quality framework for the purpose of auditing, evaluating the Endocrine service and outcomes of treatment including patient reported outcomes.
  • To attend consultant outpatient clinics to support patients through their treatment pathway.
  • Play a central role/keyworker in multidisciplinary team meeting as the patient’s key worker and advocate.
  • Liaise with the home care teams, pharmacy and external providers to support the provision of treatments for Endocrine treatments, i.e: Growth hormone
  • Update and maintain the Endocrine patient database.
