Colorectal / Stoma Care Clinical Nurse Specialist

Job overview

The colorectal stoma care Nurse Specialist (CNS) will act as the clinical expert and professional resource to the multi-professional team, patients and carers within their specialist service. The post holder will take overall responsibility to lead, manage and develop this specialist area of practice. This will include the management of a caseload of patients, as appropriate. The post holder will have an important role with patients, their families and other clinicians raising awareness of signs and symptoms of bowel disorders and disease and the possible diagnostic and treatment options. His or her own working practice should be developed to a level whereby the nurse specialist can directly or indirectly influence all aspects of care and management of patients with colorectal disorders in particular those who have or due to have stoma formation. This role will necessitate carrying out colorectal cancer follow up.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. 

Main duties of the job

  • Demonstrate clinical knowledge and interpersonal skills in caring for patients with colorectal cancer and colorectal diseases with a particular emphasis on stoma care.
  • Participate in establishing and running colorectal nurse led virtual colorectal cancer follow up clinics, pre-admission clinics and attend joint new patient and follow up clinics. 
  • During pre admission clinic, where appropriate offer Health Needs Assessment and referral to supportive cancer services in addition to carrying out pre operative stoma counselling and siting
  • Demonstrate clinical decision-making skills in referring patients for further tests or discharge to GP.
  • Work towards developing advanced practice functions including clinical history taking and physical examinations skills. 
  • Co-ordinate the patient care pathway
  • Provide continuity of patient care on a daily basis by following the patient’s journey from pre admission/admission, discharge and follow up/continuous support and encourage ERAS principles during the patients hospital stay.

Detailed job description and main responsibilities

  • Participate in the process of ensuring patients receive sufficient information to enable informed consent by communicating complex information regarding management of care, including: benefits, risks, (i.e. stoma herniation risks) alternatives complications and lifestyle advice.
  • Review medication and recommend changes in conjunction with consultant colorectal and / or gastroenterolgists  depending on individual needs.
  • Maintain patient records in a concise and literate manner ensuring hand over of complex management issues to the relevant healthcare professional.
  • Liaise with Consultant colorectal surgeons, Registrars, secretarial staff, endoscopy and radiology Teams in addition to ward staff regarding patients clinical care needs to promote effective communication related to patient clinical needs and condition prior to procedures and afterwards.
  • Provide specialised advice which contributes to the care and education of patients/carers within the speciality of stoma care/ fistula care/ cancer care.
  • Seek expert help or more experienced help where necessary.
  • Record all patient information using relevant manual / computerised systems.
  • To maintain a professional attitude at all times, developing the role of the nurse in the best interests of the patient and profession in accordance with the NMC Code of Conduct.
  • To integrate into the existing clinical structure and to support other professionals to manage patients with colorectal disorders.
  • To promote effective communication with all members of the multiprofessional team in relation to patient management.
  • To participate in research and audit.
  • To take responsibility for adjusting one’s own SCOPE of professional practice in order to demonstrate competence and continuous development in line with patient and service needs.
  • Undertake teaching and act as an educational resource for health care professionals.
  • Establish links and networks with other colorectal healthcare professionals, responsible for care of this patient group at a local, regional and national level.
  • Use every opportunity, both formal and informal, to increase understanding of all staff groups regarding recognition and care of the colorectal patient.
  • Where required work with patients to devise suitable educational and information materials regarding stoma care.
  • Share expertise at local, regional and national events.
