Disclosure and Barring Service (DBS) Checks
Many of the roles we advertise have a requirement for a DBS check to be carried out. If you are successful for a role and it requires a DBS check the Trust will carry out the initial check and subscription to the DBS Update Service, however successful candidates will be expected to cover the associated costs . Probationary Period
All new staff to the Trust will be subject to a standard 6 month probationary period, the details of which will duly follow in your contract of employment.

Agile and Flexible Working
All our staff will have a work base. However the Trust operates as an Agile Working Team therefore staff may be working from home on a regular basis as required by the service. However, occasionally travel around Warwickshire and into your base will be required so access to your own transport would be helpful.

We are looking for Advanced Clinical Practitioners (Band 8a) and Trainee Advanced Clinical Practitioners (Band 7) to join our Out of Hospital Urgent Community Response team in South Warwickshire University NHS Trust (North Warwickshire based team). The Urgent Community Response teams provide urgent care to deteriorating patients within their own home/care homes and the Virtual Ward, as well as working closely with the Medical Consultant led Frailty Assessment Unit in George Elliot Hospital. The role will be an opportunity to work in a well-established dynamic team.
This is an exciting time to join our team as we continue to develop responsive services to assess, diagnose and treat unwell frail patients at home, and would suit those from a range of backgrounds with experience in acute / emergency care, care of the elderly or community / primary care services. An understanding of Frailty and the need for admission avoidance is key.
Main duties of the job
To deliver a clinically autonomous Advanced Care Practitioner led service in the Community.
The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care demonstrated through their professional Advanced Competency framework.
To work closely with Medical and Nursing staff in both primary and secondary care, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.
To work autonomously assessing individual patient needs, initiating investigations, determine a differential diagnosis and initiating appropriate holistic, research based treatment with the development of a personalised clinical management plan, including independent prescribing.
To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.
Monitor and lead improvements to standards of care through, supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.

Leadership & Consultancy:
The post holder will be expected to:

Clinical Direct Care Practice:
The post holder will:
Be a champion for the care of the frail older person.
Practice autonomously and self-directed.
Hold high-level MDT working skills to support others with extending their professional roles.
Provide health promotion and prevention orientation, and comprehensively assess the patients for risk factors and early signs of illness.
Have high-level condition-specific clinical knowledge and skills.
Provide high-level assessment, diagnosis, planning and implementation programmes of evidence based care for the patient group, and be able to reevaluation implemented treatment plans as appropriate.
Assess and manage complications or exacerbations of LTC, this may require additional diagnostic and treatment skills.
Ensure interventions are taken in line with best practice guidance.
Undertake advanced skills appropriate to role, ensuring clinical competencies are maintained in accordance with a framework for advanced practice.
Provide advice, information and support for Trust staff caring for specific client groups.
Prescribe within the boundaries of extended non-medial prescribing.
Provide advanced knowledge advice and support to client groups.
Deliver technical skills required for intervention for specialist patients.
Identify local, demographic, cultural and environmental factors that influence the health and quality of life for client groups.
Contribute to the coordination and effective management of the admission and discharge process.
Ensure child protection and vulnerable adult legislation is adhered to within own practice and service.

The post holder will:
Contribute to the formal evaluation of the advanced practitioner role to develop new knowledge or translate new findings in to local practice.

Evaluate clinical practice through audit of service, including patient satisfaction, developing and managing strategies to address any shortfalls.

Physical Dimensions:
Ability to drive is essential.
Lifting and carrying of equipment during home visits.
There is potential exposure to infection in patient settings. There is a requirement to follow infection control procedures.
There is a requirement to use personal computers on a daily basis.
There is a requirement to be in frequent, direct contact with patients who are acutely ill, infectious and being actively treated for medical conditions. The post holder may be required to directly deal with relatives who are highly distressed and who may exhibit verbal or challenging behaviours. There may also be highly emotive situations to deal with e.g. decisions around DNAR, preparation for end of life and bereavement support for families.
Effort and Environment:
The following information has been designed to assist the recording of the effort and environment factors required for Agenda for Change.
Physical Effort: Some moderate physical effort required regularly, every week for clinical examination in constrained environment on community hospital wards, patient homes and FOPAL department. Examination of potentially heavy limbs necessary.
Mental Effort: To work in an environment with constant interruptions for staff and patient needs and from others dealing with service issues. Concentration required for clinical assessment, and making clinical decisions.
Emotional effort required in the job:
Delivery of unexpected bad news to patients and relatives, though unpredictable, occurs quite often.
