Job overview

The Severe Mental Illness Local Commissioned Service (SMILCS) have a vacancy for 0.8WTE (4 days per week) Band 6 registered mental health nurse.

We are looking for a highly motivated, enthusiastic, creative, flexible and resourceful qualified nurse to join our team of community mental health nurses. A passion for working with people to support individual recovery goals and to work collaboratively with patients, carers, GPS and ATS staff is essential. You will also need to forge close links with other community resources and stakeholders.

After a period of treatment, many clients with severe and enduring mental health needs will be sufficiently stable to be discharged back to primary care from secondary care services. SMI LCS enables the ongoing care and support of these patients in the community and facilitates the throughput of people between primary care and specialist services.

Main duties of the job

Each person discharged to SMI LCS have a designated nurse to support them.  The role includes provision of clinical support, guidance and training to G.Ps and practice staff involved in the medical management of SMI LCS patients, provision of specialist mental health step-down support to patients discharged from specialist Mental Health services to SMI LCS, improving the integration of physical and mental health care and personalised recovery orientated care planning for patients.

Qualifications

Essential criteria
  • Registered Mental health Nurse with current registration
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development

Knowledge/Experience

Essential criteria
  • Significant post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of clinical governance
  • Previous experience of working in the community
