Job overview
If you enjoy the complexity, challenge and fast pace of working in acute mental health this role is for you. As a member of the multi-disciplinary Mental Health Liaison Team at St Richards Hospital in Chichester you will be supporting people experiencing a Mental Health crisis in addition to those who may be experiencing a relapse in their diagnosed mental illness. A number of our patients are referred with self-harm or a history of self-harming behaviours. MHLT also receives referrals from patients admitted to the wards with complex physical and mental health problems.
The role involves working alongside the acute hospital colleagues and health professional including drugs and alcohol services, Social Services and charitable organisations involved in the patients care and ongoing support.
The team operates the Core 24 Liaison Model and is a 24/7 service. The urgent care practitioner would work day and night shifts. The Working Adults and Older Persons teams are integrated. The Liaison Team are commissioned for two Older Persons Liaison Nurses Monday to Friday and their speciality is working with Older Persons Mental Health
Within the multi-disciplinary team, you would be working with the Team Leader, Clinical Nurse Specialist, Liaison Consultant Psychiatrist, Counselling Psychologist, Urgent Care Practitioners and Older Persons Mental Health Liaison Nurses
Main duties of the job
The role of this team is to provide full biopsychosocial assessments and risk management plans for patients with mental health needs. Then to establish client centred safe discharge plans or signposting the patients to others appropriate services. The successful candidate will be caring for patients who have both physical and mental health needs.
The post involves liaising with other mental health services and agencies, in addition to acute inpatient units. It is preferable to have previous experience of working in acute care (inpatient setting/crisis team/mental health liaison team). In the post you will be working flexible hours to enable the team to provide a 24/7 service in line with Core 24.
You will receive regular supervision and have the opportunity for personal and professional development as well as contributing to the team's development. The team has access to a psychologist and monthly reflective practice.
This is a seven day per week service from which will necessitate some shift work and unsocial hours with enhancements.

- Knowledge of MHA (1983/2007).

- Demonstrate ability to enhance care through evidence-based knowledge
