Job overview
The post holder will be part of the Community Cardiac Rehabilitation Service, based within the Calderdale Cardiac Rehabilitation Service. They will carry out administrative duties to support the Cardiac Rehabilitation Team and contribute to the delivery of cardiac rehabilitation across Calderdale. In addition, they will provide highly skilled support to individuals who have been referred to the Cardiac Rehabilitation Team.
The work will involve assisting with the Cardiac Rehabilitation groups, contacting patients, and assisting with the office administration for the Cardiac Rehabilitation Service.
The post holder will (in allocated time) act as a point of contact for all communication for the team.
Main duties of the job
The post holder will be working under indirect supervision of the registered practitioners in the team. This will involve taking responsibility for an identified and delegated workload following screening and /or assessment by a registered practitioner. The post holder will undertake delegated work following a rehabilitation plan devised by the registered practitioner and will be expected to make minor adjustments to this following agreed protocols and guidelines, liaising with the registered practitioner as necessary. Work will be carried out in the Cardiac Rehabilitation Base and community venues.
- Assisting with exercise assessments of cardiac patients
- Assisting with and or completing the exercise assessments at point of discharge
- Monitoring progress and feeding back to the registered practitioner.
- Making minor adjustments to treatment programmes within pre-determined limits
- Assisting registered practitioners with complex treatments.
- Assisting with group exercise and advice sessions.

Detailed job description and main responsibilities
· To utilise the SystmOne clinical information system to retrieve patient information, register referrals and discharges, deal with tasks, type and send letters.
· prepare responses/letters/acknowledgments and requests for the Cardiac Rehabilitation clinical lead and therapy staff.
· To ensure written documentation is accurate and disseminated to other professionals within the Trust, external agencies and carers
· To provide a first point of contact for service users, answering the telephone and dealing with enquiries in a sensitive manner to document and convey messages to the team.
· To arrange appointments for clinic consultations, home visits and meetings face to face and/or virtual.
· To process audit statistics for relevant personnel and assist with inputting audit data onto the ongoing national audit for cardiac rehab
· To check email accounts, process referrals and amend waiting lists for cardiac rehab
· If required to be present at long covid MDT meetings to take notes and complete templates within patients notes.
· To print patient assessment packs including exercise sheets and information booklets
· To set up and clean down the group venues both before and after groups.
Person specification
QUALIFICATIONS / TRAINING
Essential criteria
- English and Math GCSE grade 9-2 (A*-E) or equivalent or high level of literacy and numeracy skills demonstrated within application
- Competence based level 2 Qualification e.g. NVQ or Diploma in Care or Care Certificate Standard – demonstrating the 15/16 standard have been meet and signed by an employer.

- Competent / able to evidence the completion of Health Care Assistant core competencies.
Desirable criteria
- Competent in venepuncture, cannulation and simple wound dressings
