Calderdale & Huddersfield NHS Foundation Trust is a leader in the digital health revolution. With the launch of a full Electronic Patient Record in 2017 the Trust aims to be the safest, most efficient and patient centred Trust in the NHS. The launch is part of a 10-15 year ongoing project of modernisation with future opportunities for research, development in this rapidly developing field of medicine. Through working at CHFT you can be part of this innovative area aiming to bring safer and more effective care to our patients.

Calderdale and Huddersfield NHS Foundation Trust Outpatient Services had over 600,000 patients visiting for appointments in various specialities 2018/19. The CQC  rating for Outpatients services  is Good and has held this rating since 2016.

As part of the NHS Digital Health Revolution we are currently transforming and modernising outpatients in line with the NHS Plan and use electronic patient record technologies. EPR was implemented in 2017 and outpatient nursing staff went live with erostering in April 2019.

We are currently looking to appoint x2 Band 5 Staff Nurses  in permanent positions at full and part time hours . The speciality areas  are CHFT Medical Outpatients Department  based at  Calderdale Royal Hospital and Acre Mills in Huddersfield. Experience of working in an medical acute ward  would be an advantage as we deal with patients with long term medical conditions ( respiratory, cardiology, gastroenterology, neurology, diabetes for example). Outpatient Department experience in a clinic setting is preferable but not essential.

You must be able to demonstrate excellent communication & documentation skills and the ability to ensure the delivery of high quality care in an ever changing NHS.

Working with a multidisciplinary team is key to success in the role.
