We’re looking for a motivated, enthusiastic, and experienced cardiology nurse to join our small, friendly team of Heart Failure Specialist Nurses at an exciting time of service expansion.

We’re a small, close knit, and supportive team who pride ourselves on working collaboratively and looking after one another.
We work closely with a wider multidisciplinary team, including cardiology consultants and a specialist pharmacist, to provide seamless, high quality care.
Currently, we cover the Calderdale community, with each team member spending one day per week reviewing inpatients remotely across CRH and HRI.
Main duties of the job
You will bring confidence in cardiology, with a sound understanding of heart failure and its management. We are committed to supporting your development and offer a wide range of learning opportunities, including:
• Heart Failure Diploma
• Independent Non Medical Prescribing
• Advanced patient assessment
• A variety of additional courses tailored to your learning needs
You will work autonomously, managing your own time and caseload to deliver high quality care to patients with heart failure. This includes:
• Initial assessment
• Medication optimisation
• Ongoing education and support to promote self management
One day per week will be dedicated to reviewing inpatients at Calderdale Royal Hospital (CRH) and Huddersfield Royal Infirmary (HRI) (currently completed remotely).
As our Hospital at Home service develops, the team will provide care 7 days a week, 8am–8pm, and you’ll have the exciting opportunity to help shape and influence the future direction of this service.
You will receive a supernumerary period and ongoing support from an established Heart Failure Specialist Nurse, ensuring you feel confident and supported every step of the way.

Detailed job description and main responsibilities
Will work across the hospital with responsibility for patients with heart failure in the inpatient, outpatient, community and Hospital at Home setting.
Will be responsible for the early identification of patients for the Heart Hospital at Home ward from the acute setting and supporting the on-going care for patients whos are actively being managed on the Hospital at Home.
The post holder will take overall responsibility to develop the specialist area of practice in partnership with other members of the specialty team (Lead clinician, Matron, service Lead etc) and will work collaboratively to develop services and quality of care delivered.
The post holder will be expected to act as the patients advocate at all times.
The post holder will carry responsibility for their defined caseloads and offer highly specialist service advise which includes setting the standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care.
The post holder will contribute to achieving the objectives of the role and work within the clinical governance framework.
Will work autonomously managing own clinical caseload and to support the Hospital at Home.
Will use relevant specialist training, knowledge, and expertise to clinically diagnose, assess and deliver evidence-based care through education and advise on self-management

QUALIFICATIONS / TRAINING
Essential criteria
- First level registration NMC/AHP registered
- Post registration experience/qualification relevant to the specialism or willing to work towards
- Teaching qualification
- Non-medical prescriber or willingness to undertake this
- Cardiology Experience essential
KNOWLEDGE, EXPERIENCE & EXPERTISE
