Job overview

An exciting opportunity has arisen for an experienced, highly motived and enthusiastic Diabetes Nurse to join our small dedicated diabetes team to deliver specialist advice, education, and services at Airedale General Hospital.

The team comprises of Diabetologists/Endocrinologists, Diabetes Nurse Specialists, Podiatrists and Dieticians. 

We have been successful in our transformation and integration with the community diabetes nursing specialist team to enhance the service we deliver to our patients. 

You must have excellent communication skills and be able to demonstrate the knowledge, experience and motivation to care for the patient living with diabetes.

The successful candidate will be able to provide direct expert diabetes nursing care for both inpatients and outpatients.

The successful candidate will also utilise desirable advanced nursing practice skills and knowledge, in particular, nurse prescribing to meet the highly complex diabetes needs of patients who will be accessing the hospital specialist diabetes services.   

Main duties of the job

·         Provide direct expert diabetes nursing care for outpatients and within Primary care-based clinic settings accessing specialist diabetes care from the Hospital Trust

·         Utilise advanced nursing skills and knowledge, in particular nurse prescribing to meet the needs of patients accessing the diabetes service.

 

·         Through clinical supervision and professional development review and maintain own levels of clinical competence and expertise

 

·         Using advanced clinical skills comprehensive and holistic patient assessments to inform diagnosis and treatment in patients with an existing diabetes or gestational diabetes diagnosis.

·         Collaboratively work with patients, carers, multidisciplinary team members, other healthcare professionals’ and agencies to ensure individual diabetes needs are met.

 

·         Establish and maintain effective communication with patients and relatives/carers.

·         Maintain accurate and concise electronic patient records.

 

·         Act as a patient advocate appropriately to support and promote their participation in decision making.

 

·         Work ethically and legally within the NMC code of Conduct and Practice.

·         There will be an expectation to support a weekend rota for service delivery, the current hours are Saturday working 8:30am – 16:30pm approximately once a month.

Qualifications

Essential criteria
  • Level 1 Registered Nurse
  • Relevant post qualification study at Master level
  • Willing to undertake non-medical prescribing qualification
  • Teaching/assessor/mentor qualification
Desirable criteria
  • Non-medial prescriber
  • X-pert educator
  • DAFNE educator
  • Insulin pump university module

Experience

Essential criteria
  • Significant experience as a Diabetes Specialist Nurse
  • Ability to work independently and autonomously
  • Practice within guidelines and protocols to ensure deliver of high quality diabetes care
  • Participation in clinical audit
Desirable criteria
  • Diabetes inpatient experience
  • Experience delivering diabetes pre-conceptual and antenatal care
