Advanced Clinical Practitioner
8A
Job overview
We are looking for an established Advanced Clinical Practitioner to support in the running and continued development of an integrated SDEC within AMU.
The unit has approximately 50 admissions a day Monday-Friday including ED, SDEC and GPs attendances and is well supported by the Acute Physicians’. We have now got the opportunity to provide a 7 day SDEC service and are expanding our ACP.
We recognise the profile of Advanced Practice needs raising within the department and a key element of this role would be in helping advocate for and building an ACP workforce to meet the future demands of the service.
Main duties of the job
To be responsible for the rapid assessment, diagnostics and treatment of patients within the Same Day Emergency Care (SDEC) and Acute Medicine setting, using an advanced level of professional accountability, autonomy and judgement. This includes highly complex decision-making across a broad range of situations/conditions.
To work autonomously, using advanced knowledge and skills, underpinned by theory and experience, to holistically assess the identified patient caseload, using a range of methods across a broad range of, often complex, patient conditions within Acute Medicine and SDEC.
To plan and manage complete episodes of care, working in collaboration with Acute Medical Physicians and others, delegating and referring as appropriate, to ensure timely, effective patient management and optimise health outcomes, in line with nationally recognised best practice.
The post holder will deliver an 80% clinical component to their role and 20% related to service development, appraisal, clinical audit, teaching, self-development and research.
To play a key operational role and maintain a pro-active approach to managing and leading clinical pathways and flow.
