Salisbury Hospital is expanding its Frailty Same Day Emergency Care (SDEC) service and we are looking for enthusiastic Advanced Clinical Practitioners (ACPs) to join our dedicated team.

This is an exciting opportunity for clinicians with a passion for caring for older people who thrive in fast-paced, dynamic environments. Working alongside Consultants, Therapists, and Community Partners, you will play a key role in delivering Comprehensive Geriatric Assessments within the Emergency Department and SDEC areas.

We are seeking motivated colleagues who value collaborative, patient-centred care and want to contribute to shaping and developing services for our older and frail population.

In this role we can offer you flexible working opportunities (We work a Mon–Fri service, with scope for varied start/finish times and part-time roles) in a supportive, forward-thinking team environment with strong multi-disciplinary collaboration.

Prif ddyletswyddau'r swydd

    • Provide advanced clinical practice and deliver Comprehensive Geriatric Assessments for older patients in ED and SDEC.
    • Utilise your full ACP skill set to assess, examine, diagnose and manage patients with undifferentiated presentations.
    • Support same day discharge and admission avoidance, working closely with community partners including Hospital@Home virtual wards.
    • Develop expertise in managing common conditions affecting older people, such as Heart Failure, Falls, and Parkinson’s Disease.
    • Contribute to service development as SDEC expands across the Trust.
    • Supervise, support and teach ACP trainees and colleagues within the multi-disciplinary team.

Qualifications

  • Previous NHS experience.
  • Registered General Nurse or Registered Allied Health Professional with current NMC or HCPC professional registration.
  • Master’s Degree in Advanced Clinical Practice or Master’s level award that covers all 4 pillars of the Advanced Clinical Practice (ACP) capabilities (essential to include: advanced clinical assessment skills and diagnostic reasoning qualifications) (Will consider applicant with all other Master’s (ACP) level modules completed but pending completion of Dissertation module)
  • Registered Non-Medical Prescriber, (if professionally appropriate) with evidence of updating knowledge appropriate to patient groups for role.
  • Post registration qualification or equivalent experience gained by undertaking on-going personal development and training focused on older adults / frailty / advanced practice skills.
  • Recent experience in managing patients with Acute Frailty presentations in an acute hospital setting.
  • Significant post registration experience at band 7 working with Older Adults.
  • Clear and demonstrable evidence of advanced, autonomous practice working with the older adult population.
  • Evidence of proactive involvement in the supervision and development of members of the multi-disciplinary team and able to demonstrate change management skills.
  • Confident in the use of computer systems, spread sheets, databases, data collection, word processing and report writing.
  • An understanding and insight into relevant, current issues and developments within the NHS, especially in relation to Frailty and Older People.
  • Ability to evaluate care leading to improvement in quality standards and service improvement.

  • Able to demonstrate clinical skills beyond traditional professional boundaries, such as prescription of mobility aids in nursing. Generic interdisciplinary experience and skills.
