Job overview
The Integrated Discharge Service at Salisbury NHS Trust plays a critical role in supporting patients to leave hospital with the support and care they need. We work hard with our partners in health and social care to ensure patients, families and carers are involved in discharge planning and strive for the best outcome for every patient.
Main duties of the job
You will work with professionals across multiple organisational and geographical boundaries to achieve good quality discharge plans, and will share your skills and experience through teaching and coaching ward staff in this specialist area. You will manage complex caseloads across adult inpatients, and will provide specialist advice and service for patients needing CHC fast track applications. You will be bold and confident, and with good humour and positivity will demonstrate the best qualities of the profession to ensure our patients experience the best our service can offer.
