Job overview

We are excited to be extending our team at Dane Garth and are looking to appoint to B3 Health Care Support Worker posts across the Access and Urgent Pathway, supporting patients both in their own homes, FGH and the Health Based Place of safety.

Main duties of the job
To work as part of a well-established exciting team within the Health Based Place of Safety team providing a high standard of intervention in line with service objectives over a 7 days week 24 hour rota supporting the urgent care pathway.
To assist the Heath Based Place of Safety practitioners to assess, plan, implement and evaluate individual care plans.
To work with Service users who are detained under a Section 136 or 135 of the MHA, who are in Crisis who have been brought into the Health Based Place of Safety by the police and are awaiting an assessment
To monitor and assess “Risks” and report back to the Mental Health Practitioner as required.
To assist Mental Health Practitioners and the wider MDT during the assessment and following the assessment to ensure a safe discharge plan is implemented
Ensure the smooth transition for service users between specialist and mainstream services and other teams.
At times when the suite unoccupied you will be expected to support the mental health liaison team with contacts and also support the acute wards within Dane Garth – the Dova Unit which is a 20 bedded mixed sex acute mental health inpatient unit and Ramsey unit a 15 bedded organic assessment unit.

Education / Qualification
Essential criteria
- An interest in completing NVQ Level III or higher in Health or Social Care.
- or QCF Diploma in Health and Social Care.
- Training or can demonstrate the equivalent skills and knowledge gained through work (paid/unpaid).
Knowledge
Essential criteria
- Demonstrate an understanding of the issues surrounding mental illness.
- Understands their duties and responsibilities under the Data Protection Act with regard to patient confidentiality.
Experience

- Working in a caring environment, e.g. nursing home, care home, hospital ward, home carer etc.
- Working as part of a team.
