An exciting opportunity has arisen for an experienced and self-motivated nurse to join the Acute Care at Home Team based at St Barnabas Hospital, Saltash.

You will be part of a multi-disciplinary team delivering services to people in the community, striving to deliver high quality personalised care.  The aim of the service is to ensure that an integrated approach is taken when delivering care.

The post will deliver a range of skills and experience to all aspects of community care and will involve working with patients in their own homes or in the Acute and/or Community Hospital settings.

 

Main duties of the job

The post holder, as a Community Acute Care Senior Staff Nurse, is responsible for the delivery of high-quality evidence-based care for acutely ill patients within their homes, with the aim of preventing hospital admission or facilitating rapid discharge.

The post holder will liaise with the referring hospital or GP when arranging admission to the scheme, and will take responsibility to effectively assess, develop, implement and evaluate patient care.

The Acute Care at Home Team work on a flexible rota shift system.  The service covers a period of 0800 hours to 2030 hours 7 days a week.  The geographical area when on duty is anywhere within North and East Cornwall areas. The post holder is predominantly a lone worker within this area.

As part of the role you will regularly deputise in the absence of Team Leader and assist in the management of the team, including the performance and development of staff and providing support to other team members.

Education and Qualifications

Essential criteria
  • RGN 1st level registration
  • Diploma level specialist training, experience, short courses (or equivalent experience)
Desirable criteria
  • Prescribing qualification
  • Guidance Score Prescribing qualification 2 Teaching qualification, eg, City & Guilds, HEA Mentorship module (or working towards)
  • 1st Degree Diploma or working towards

Experience

Essential criteria
  • Working without direct supervision, yet as a team member through experience and education
  • Substantial post registration experience in Acute or Community Team
  • Experience of a wide range of medical conditions
  • Evidence of autonomous practice
Desirable criteria
  • Evidence of good medical grounding within an acute setting
  • Experience of audit

Knowledge and abilities
