Job overview

This post is Fixed term/Secondment for 4 months to meet the needs of the service. The position to be reviewed March 26 and extended to 31/3/27

The ability to speak Welsh is essential for this post 

To assist the GPs and practice nurse team in the service and delivery of the care management of the practice population.

Responsible to: Clinical Co-ordinator, Operations Manager and clinical Key Teams.

The post holder works under indirect supervision of the Health Visiting team manager and undertakes task and duties delegated by the health visitor or a suitably qualified regulated professional. They will work collaboratively with the general practice team to meet the needs of patients, following policy and procedures.

Main duties of the job

  • Communicate effectively with patients and carers, recognising their needs for alternative methods of communication.
  • Undertake effective communication with patients in specific clinical situations.
  • Communicate effectively with other team members.
  • Personal and people development.
  • Take responsibility for own developmental learning and performance, including participating in supervision.
  • Take responsibility for maintaining a record of own personal development.

Essential criteria
  • NVQ Level 3 or working towards this or equivalent qualification
  • Previous experience working in a general practice or other healthcare environment
  • Able to make judgements where there is more than a straightforward choice of options, e.g. new patient health checks, urinalysis etc
  • Demonstrates an ability to value the opinions of others Demonstrates high level of self awareness, i.e. strengths and weaknesses, personal qualities and skills Willingness to undergo education/ training for both practice and service needs
Desirable criteria
  • HCA course specific to general practice
  • Following appropriate education and training has the ability to undertake the administration of influenza both intramuscular and intranasal, pneumococcal vaccine and shingles vaccine. Also the administration of hydroxocobalamine (vitamin B12) injections
  • Ability to mentor new HCSWs or junior staff to the working environment
