Job overview

To support adults with Learning disabilities (LD) who present with complex physical health needs, this may also include an element of behaviours that challenge and/or Mental Health needs.

To work within the multidisciplinary team (MDT) towards a person centred approach to formulate, implement and evaluate appropriate care plans to improve positive health outcomes.

Be responsible for undertaking specialist nursing assessments to support MDT intervention and person centred goals.

To work as part of an MDT to develop, deliver and implement resources and training to mainstream services to promote inclusion and improved health outcomes for people with LD

Main duties of the job

  • To maintain an active caseload of service users, working jointly with mainstream services, to promote better access to health care and improved outcomes.
  • To give nursing support and advice in relation to health care to people with an LD and their families/carers.
  • To participate in assessment, planning, implementing and evaluating of MDT interventions.
  • To undertake, record and review risk assessments pertaining to the service user and the environment.
  • Prioritise workload in order to deliver a fair and equal service-based on clinical need – with support as required.
  • To work autonomously and organise workload – with support as required.
  • To utilise and participate in regular clinical and peer supervision using reflective practice in accordance with clinical governance.
  • To develop and maintain positive therapeutic relationship with service users and carers.
  • Contribute to service user formulation and safeguarding adults meetings.
  • Participate in multidisciplinary (MDT) initial assessments to identify client eligibility and need.
  • Have an awareness of the current health inequalities experienced by people with an LD in order to inform service development.
  • As part of an MDT develop, pilot and deliver training to GP’s, and other mainstream health services to promote and support the implementation and use of reasonable adjustments.

Skills, Knowledge, Experience

Essential criteria
  • Effective communication skills with clients and carers, colleagues, other statutory, private and voluntary agencies using a wide range of communication methods including written, verbal and electronic formats.
  • Experience of working with people with LD and their carers’.
  • Experience relevant to community working
  • Experience of developing and delivering training
  • Effective report writing skills, you will need to write clear, factual and accurate reports to provide specialist information
Desirable criteria
  • Able to work autonomously, whilst able to prioritise and plan own workload and others workload.
  • Experience of implementing new policies, procedures and ways of working
  • Knowledge of clinical governance
