Job overview

The Acquired Brain Injury (ABI) service is seeking a proactive and dynamic individual with strong communication skills to join the team as a case manager. The ideal candidate will have experience working with individuals with an acquired brain injury and a genuine passion for supporting their recovery and well-being.

The role is suitable for an experienced band 6 practitioner looking to take the next step or someone already in a band 7 role who would like a new opportunity.

The post is based primarily within the Wirral team of the ABI Service which is an established and successful multi disciplinary community based team covering  Wirral and Cheshire.

We provide comprehensive support for individuals affected by acquired brain injury, including case management, vocational rehabilitation, and neuropsychological interventions. 

The case manager role offers the opportunity for blended working, including face to face sessions, remote sessions via telephone and video and working from home. It will involve travelling around the Wirral.

The service collaborates with universities to facilitate student placements, support quality improvement projects, and promote continuous learning opportunities.

Main duties of the job

The case manager will be an experienced therapist or healthcare professional who will proactively coordinate interdisciplinary collaboration between statutory and independent provider services to address the needs of clients with acquired brain injuries living on the Wirral.

The postholder will take lead responsibility for the co-ordination and delivery of initial screening, risk assessments and evidence based interventions for all community patients.

Qualifications

Essential criteria
  • Degree or diploma in clinical specialism
  • HCP or equivalent registration
  • Demonstrate learning at a Master’s level or equivalent experience in the specialist area.
  • Member of professional body
  • Evidence of a range of specialised post- graduate courses
Desirable criteria
  • Leadership qualification
  • Educators qualification

Knowledge & Expertise

Essential criteria
  • Extensive knowledge of acquired brain injury therapy and management
  • Knowledge of multidisciplinary working, risk assessment and goal setting
  • Knowledge of National & local legislation, policies and procedures and guidelines.
  • Experience of working with other agencies e.g. care agencies, private providers, voluntary sector
  • Ability to manage own caseload and work as part of a multi-disciplinary team.
  • Knowledge of safeguarding policies and procedures
Desirable criteria
  • Knowledge and experience of development of patient pathways
  • Knowledge and experience of working with the Mental Capacity Act, Equality Act and other relevant legislation.
  • Knowledge of a range of standardised cognitive assessment tools relevant to ABI
  • Specialist knowledge of provision of services in local economy.
  • Evidence of leadership and implementation of quality improvement processes.

Experience

Essential criteria
  • Minimum 5 years post graduate experience, including 2 years as a specialist practitioner in the area of rehabilitation/ABI/community.
  • Specialist clinical knowledge of rehabilitation and community therapy practice
  • A broad experience across a variety of different areas and settings
  • Experience of leading case conferences and family meetings
  • Be able to prioritise work and adapt to change in priorities and manage unforeseen situations.
  • Knowledge of standardised cognitive and perceptual assessments and interventions
  • Experience in quality initiatives.
  • Experience of completing risk assessments.
Desirable criteria
  • Experience using/ interpreting standardised cognitive and perceptual assessments and interventions
  • Experience in staff appraisal and supervision
  • Demonstrate an ability to facilitate presentations and provide staff training
