Job overview

West London NHS Trust is seeking a skilled and motivated Nurse or Physiotherapist to join the Hounslow Community Respiratory Service. This Band 6 role offers an exciting opportunity to deliver specialist, patient-centred rehabilitation for adults with long-term cardiac and respiratory conditions, including those diagnosed with Long COVID, across home, clinic, community venues, and virtual platforms.

The successful candidate will provide holistic assessment, rehabilitation, and case management to support patients in maximising independence, functional ability, and quality of life. You will work collaboratively within a multidisciplinary team to support Pulmonary and Cardiac Rehabilitation (PR/CR), Early Supported Discharge (ESD), chest physiotherapy, and specialist interventions delivered through HOSAR.

Main duties of the job

·       Delivering Pulmonary and Cardiac Rehabilitation programmes in line with NICE, BTS, and ACPICR guidance.

·       Providing specialist assessment and case management for adults with long-term cardiac or respiratory conditions.

·       Supporting patients with breathlessness, fatigue, anxiety, and functional limitations.

·       Delivering chest physiotherapy and oxygen therapy interventions within the community.

Essential criteria
  • Registered Nurse (NMC) or Physiotherapist (HCPC).
  • Post-registration experience in community or acute care, including MDT working and rehabilitation.

Skills and Experience

Essential criteria
  • Skills in advanced assessment, rehabilitation planning, and patient education for long-term cardiac and respiratory conditions.
  • Experience in primary care or integrated care settings.

Knowledge

Essential criteria
  • Knowledge of post-COVID pathways and HOSAR service delivery.
