Job overview
The Senior Clinical Urgent Care Navigator/ Gatekeeper will report directly to the Senior Clinical Urgent Care Navigator/ Gatekeeper, the post holder will have highly developed specialist assessment skills and knowledge of Urgent Care Services.
The post holder will work across the Trust to develop the assessment capacity and capability for Urgent Care. They will provide practical and mentoring guidance to the clinical and operational teams in developing best models of care in line with best practice.
Main duties of the job
The post holder will lead on the development of partnerships to achieve the best models of care, ensuring robust assessment processes, safe waiting and monitoring and evaluation methods are in place to support
sustainable services supporting the reduction on Out of Area placements.
The post holder will align themselves with the Acute Providers, Local
Authorities, Police, Housing, VCSE and Commissioners to develop effective working relationships and provide leadership to the clinical and operational teams supporting positive risk taking and promotion of the least restrictive options.
