Job overview

We have an exciting opportunity for a Registered nurse or Paramedic to join The Joint Emergency Team [ JET] , to support with urgent community 2 hour response for patients in their own homes. Advanced Clinical skills and community experience working autonomously  are essential.

You will be required to work over a 7 day working week, including weekends and Bank Holidays. Hours- 07.30-20.00 hrs and 09.00 - 21.30 hrs. We are looking for someone who is hardworking, motivated with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic/clinical assessment .

 

Main duties of the job

Supporting patients in their own homes working with a multidisciplinary team with our system partners in the Urgent Community Response [ UCR] , preventing ED attendances and unnecessary hospital admissions.

The role is to work collaboratively with other team members offering holistic, high level Clinical assessments and reviews for patients who may be in crisis. To act as patients advocate facilitating choice and patient empowerment. To provide evidence based clinical/therapeutic interventions based on best practice. To work with all health professionals to provide a seamless, integrated service to our service users. To triage referrals on a rotational basis for  the Urgent Community Response Team and the Urgent Emergency Care Hub [ UEC Hub].

  •  To provide a high quality of care to patients who have a wide range of physical needs affecting their ability to maintain a level of independence. With an age range of 18 years and upwards with long term conditions.
  • To deliver patient centered care that empowers individuals to achieve their optimum physical, psychological, social and emotional wellbeing.
  • To provide a holistic and clinical assessment, investigate, diagnose and implement patient centered care working in partnership with the MDT.
  • To provide advice and support to patients, families, carers and voluntary agencies regarding the patients medical condition ensuring that an effective communication pathway exists.
  • To support and help the JET Practitioners with ensuring the delivery of a safe effective service.
  • To provide professional and clinical support acting as a role model for colleagues.
  • To provide clinical supervision and undertake appraisals.
  • To carry out complex clinical triage. Providing assessment, advice and information whilst utilising professional judgement, decision making and referring to other agencies/services as appropriate.

Person specification

Education

Essential criteria
  • RN Degree/Paramedic qualification in adult physical health
  • Mentorship Qualification
  • Evidence of Continued Professional Development
  • Advanced Clinical assessment qualification or equivalent
Desirable criteria
  • Independent Supplementary Prescribing
  • Experience of teaching in clinical practice

Experience

Essential criteria
  • Experience of complex care planning and MDT/Long term conditions
  • Evidence Autonomous practice
  • Extensive post qualification experience within rapid response/ED or similar
  • Experience of Triaging
Desirable criteria
  • Experience of working in the community
  • Experience of working with people with Long Term conditions

Knowledge and Skills

Essential criteria
  • Effective communication and IT
  • Able to calmly manage complex and stressful situations and adapt accordingly
  • Able to priorities work load for self and others
  • Decision making, delegation and problem solving skills
