Job overview
Provide a specialist nursing service for any patients referred to the Trust for vascular surgery ensuring the highest standard of nursing care
Be responsible for identifying and influence patient care through expert clinical practice, management, leadership, education, research, audit, problem solving and professional activities that will improve the provision of care for patients referred to the vascular team.
Work collaboratively within the Vascular Hub and Spoke Addenbrookes team, and as a member of the multidisciplinary team to develop the role of Clinical Nurse Specialist in Vascular. This will include completing the Advanced Skills in Clinical Assessment and Non-Medical Prescribers courses.
To work closely with the Addenbrookes Vascular Consultants, wider multidisciplinary teams including wards, vascular lab, palliative care, and community teams, to prepare patients for treatment/management and also for on-going patient support.
Independently provide advice, information and support to patients, carers and staff regarding treatment plans and diagnosis.
Main duties of the job
· Uses specialist knowledge and skills to integrate assessment findings, plan, implement and evaluate individual programmes of care for vascular surgery patients in an inpatient and outpatient setting during contracted hours.
· Establishes effective working relationships with other care professionals whose role impinges on the service i.e. in patient teams, diabetic foot service, radiology and theatre teams as well as working with other nurse specialists within the vascular network
· Demonstrates specialist knowledge and skills in the speciality and shares this with others to positively influence patient care
· Challenges poor practice and inappropriate intervention in relation to specialty
· Leads practice development/improvement initiatives relates to patient journey
· Improves effectiveness and management of surveillance programmes for common vascular pathologies and treatments
· Provides specialist advice and education for patients and relatives/carers
· Develops education programmes for patients/carers to promote their understanding and evolve strategies for self-management
· Attends MDT meetings and communicates outcomes to patients and other team members when appropriate

Essential criteria
- Registered General Nurse
- Degree in Nursing
- Research module
- Teaching and mentoring qualification
- Evidence of continuous professional development and portfolio of learning
- Leadership or management module
- Willingness and ability to work towards masters degree

- Independent Nurse Prescriber
- Advanced communication skills or counselling qualification
Experience

- Significant clinical experience as nurse specialist in General Surgery/Vascular services
