This is an exciting opportunity to be part of the complex Discharge Planning Team, providing a link between health and social care.

The role provides the opportunity to work with the MDT and our service partners in best practice, while providing expert advice on the management of patients with complex discharge planning needs.

We are  looking for an experienced nurse who has communication skills and has the confidence to both challenge practices and negotiate outcomes for our patients  to facilitate timely and safe discharges.

You will be proactive and a good problem solver with the patient at the heart of what you do.

If you are organized , able to prioritize your workload  with the ability to work in a busy team while learning a new role,  this may be the opportunity for you.

Main duties of the job

- The post holder is responsible for the provision of high quality, patient focused care
- Responsible for assessing, developing and implementing specialist Nursing care programmes
- Responsible for providing specialist advice and support to,
and liaising with, other staff

  • This post is subject to a satisfactory DBS clearance and as a result, you will be required to pay the associated fee of  £40.00. This fee will be deducted from your second month’s salary.
  • Please note:- Internal applicants on permanent contracts can only apply for this post as a secondment and must have the approval of your current line manager before applying.

  • 1. Registered nurse. 2. Relevant experience in surgery 3 Evidence of continuous professional development 4. Teaching or mentoring qualification 5. Post registration qualification in speciality or willingness to complete
