Older Adults Mental Health Care Coordinator


The older adults CMHT is keen to continue to develop high calibre services in the community for our client group, which reflects our commitment to quality of care in Bromley.

We have recently reviewed the structure of our provision and agreed a focus on developing services that meet the needs of the population and our G.P’s. and commissioning groups in the most effective way.

In order to take this exciting development forward, we are looking for a health or social care professional with exceptional organisational and communication skills who will act as care co-ordinator in the older adult’s community team within the directorate. The post holder will be a mental health professional registered with NMC or HCPC with significant post qualification experience (minimum 2 years), working in either an inpatient or community setting.

An essential requirement will be the ability to manage risk and prioritise, co-ordinate and liaise with professionals in the team and other areas in line with best practice. This is an exciting opportunity to be involved with the development of the service and requires a commitment to integrated working.
