An exciting and challenging opportunity has arisen to join our small, friendly and dedicated team within Liverpool University Foundation Trust as a Band 6 Sarcoma clinical nurse specialist working with the multi-disciplinary team within the Orthopaedic Sarcoma Service. This is as  a result of the promotion of our current post holder. 

This role is varied, interesting and crucial to ensuring an excellent patient experience every step of the way. This role will support the  Clinical Nurse Specialist to ensure that the patients receive the on-going cancer support and holistic follow-up care. 

Main duties of the job

The post holder will work alongside the 2 sarcoma consultants, and the multi-disciplinary team,  supporting patients through their on-going journey and follow up care.  The post holder will work as part of a team of Cancer CNS’s to develop the specialist area of practice in partnership with other members of the speciality team (Lead Clinician, Matron, Head of Operations), and will work collaboratively to develop services and quality of care delivered

Detailed job description and main responsibilities

Perform specialist personalised, assessment and higher level discussion of patient’s holistic needs, resulting in personalised care planning and evaluation of care delivery using advanced clinical knowledge and clinical skills.

Demonstrate higher level clinical care by managing a patient caseload providing expert assessment, planning and evaluation.

support the  nurse-led clinics (within Speciality).

Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements, including providing access to community cancer support.

Establish and maintain effective communication with patients and carers / relatives.

Support patients and their families and ensure that they receive the required information to enable them to self manage, lead or participate in their care delivery.

Recognise changes in patient’s condition that require the intervention of others and case manage/refer on as appropriate.

To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management.

To support the specialist nursing contribution to the service, in the delivery of high standards of patient care and the specialist nursing contribution to the service, work in partnership with lead clinicians, Lead Cancer Nurse, Head of Operations, and Matron to ensure the delivery of high standards of patient care.

 Work with the Band 7 Sarcoma specialist nurse and Matron to promote the development of nurses undertaking the CNO ‘Ten Key Roles’ as considered appropriate e.g. Nurse Prescribing.
