An exciting opportunity has arisen to work as a reviewing nurse as part of the reviewing team for Continuing Health Care (CHC) in Calderdale CCG. This vacancy has been created by the promotion of the previous post holder to a lead nurse post within the CHC team.
This is an opportunity to be part of a team with high standards and established processes for reviewing individuals in receipt of health care funding. It is an ideal step for those who wish to pursue career advancement within Primary Care.
The successful candidate will be supported by the team for an induction period and will shadow other team members to assist with the understandings of other processes, roles and responsibilities within the service.
The post holder will be encouraged and supported to apply and increase their skills and clinical knowledge in holistic assessments. Also, the post holder will have the responsibility for reviewing the health needs of individuals either in their own home or in a residential setting. In addition,they will also be responsible for ensuring that the current placement/package of care meets the needs of the individual and will review their eligibility for CHC funding. Moreover, the post holder will work with CHC team, key stakeholders and providers to ensure that standards of care for all individuals are delivered in accordance with their contracts and service specifications.
Evidence of up to date clinical practice, and experience of providing high standards of care will be required. In return they will be part of a committed, positive and forward thinking CHC team.
Successful candidate will have opportunities not only for advancement but to widen the breath of their holistic knowledge working across interdisciplinary services.
